Medical records

We take great care to keep your medical record confidential and secure. You can request and access your medical record through MyChart®, our secure patient portal, electronically, or by mail/in-person if you don't have a MyChart account. 

To request your medical record be released to MyChart, log in and go to "Your Menu," and search "Medical Record Request."

Patients, patient representatives, physicians and legal requestors may request information electronically by visiting verisma.com/endeavorhealth.

To request your medical record by mail/in-person and/or to obtain your radiology images/CDs, walk-in to the Medical Records Department at your affiliated hospital Monday - Friday from 9 am - 5 pm (CST). Mailing addresses are also provided below.

Bring the completed Authorization to Use and Disclose Health Information form (or complete it at the time of your in-person request). If you are picking up a radiology CD for someone else, the form must be completed and signed by the patient with your name in Section 4 before you come to the hospital. Please bring your driver’s license or photo ID.

Note: Off-site locations cannot burn radiology CDs (the service is only available at Edward Hospital or Elmhurst Hospital).

To obtain a copy of a birth or death certificate, contact the Illinois Department of Public Health.

To check the status of a Social Security card for your baby, contact your local Social Security office.
