Medical records

We take great care to keep your medical record confidential and secure. You can request and access your medical record through MyChart®, our secure patient portal, or by mail/in-person if you don't have a MyChart account. 

To request your medical record be released to MyChart, log in and go to "Your Menu," and search "Medical Record Request" or select “Document Center” and click on "View, Download, or Send Visit Records." Note: radiology images cannot be sent via email or to MyChart – see below.

To request your medical record by mail/in-person and/or to obtain your radiology images/CDs, walk-in to the Medical Records Department at your affiliated hospital Monday - Friday from 9 am - 5 pm (CST). Mailing addresses are also provided below.

Bring the completed Authorization to Use and Disclose Health Information form (or complete it at the time of your in-person request). If you are picking up a radiology CD for someone else, the form must be completed and signed by the patient with your name in Section 4 before you come to the hospital. Please bring your driver’s license or photo ID.

Note: Off-site locations cannot burn radiology CDs (the service is only available at Edward Hospital or Elmhurst Hospital).

To obtain a copy of a birth or death certificate, contact the Illinois Department of Public Health.

To check the status of a Social Security card for your baby, contact your local Social Security office.

What other health information can I access through MyChart?

Through MyChart, you can also view your test results, medications, allergies, immunizations, medical history, current health issues and due dates for preventive care. You can also view and print your COVID-19 vaccination status and any COVID-19 test results (name will appear on result). Learn how here.

Who is authorized to sign for the release of medical records?

  • The patient or the patient’s legally authorized representative (legal documentation is required)
  • The parent of a patient under the age of 18
  • The parent and teenager (ages 12-17) for records that contain mental health or other protected diagnoses contained in the health information
  • A legally-appointed executor or administrator of an estate if the patient is deceased (legal documentation, court order or small estate affidavit, and a death certificate are required)

How can I share my medical record with other healthcare professionals using MyChart?

Share My Record – This feature is used by patients to provide another person a one-time-only login to their MyChart account. This is typically used when the patient wants to share their record with an outside professional.

  • The MyChart account holder generates a single use code within MyChart and they provide the code and the website to the person they are giving access to.
  • The outside provider opens the website, enters the code, and can the view/print records.
  • This feature enables access to any data that is within MyChart.

Download My Record

  • MyChart users can download records and print or store the data to a thumb drive or electronic file.
  • A single encounter, multiple encounters, or all encounters can be chosen for download.
  • The download will just include items that are available within MyChart.

Happy Together – Links medical records from other healthcare systems or facilities that have an Epic electronic medical record.

  • When records from outside organizations are linked using Care Everywhere a patient can the link their MyChart accounts from each organization.
  • Once linked, when the patient logs into MyEEHealth (MyChart) they will see the MyChart data from the outside organizations as well as their Edward-Elmhurst Health data.
