How do I get my prescriptions refilled?
In order to best serve your needs, please call for prescription refills before your supply runs out or call your pharmacy with the name of the medication you need and ask them to send a request to us electronically. This method is the most efficient and will allow us to process your refill quickly. If your pharmacy is unable to send electronically, they will fax your request. Phoning the pharmacy for your refill expedites the refill process. Prescription requests can also be completed by using MyChart.
How do I notify my doctor in an emergency?
In the event of an emergency, please proceed directly to the Emergency Department at Elmhurst Hospital, located at the corner of York Street and Roosevelt Road, 155 E. Brush Hill Road Elmhurst, IL 60126.The ER phone number is 331-221-1000.
The Emergency Department physician will notify us directly, and we will help make the appropriate decisions regarding your care.
How do I get a copy of my medical records?
If you or your family would like to send a compliment or are concerned with your care or service, you may call us at 331-221-1115.
This is an Elmhurst Hospital outpatient clinic. It is important to note that you or your insurance carrier will receive two bills for today’s visit that, combined, represent the total cost of your visit. One bill will be for the professional or clinical fees from the visit and the other bill will be for the facility fee of your visit. These bills will be mailed separately and may not be received at the same time. If you have any questions or need assistance regarding your charges, please call the phone number listed on the bill.

Hospital services will be billed by Elmhurst Hospital. Please ask your insurance company about how they cover bills for hospital-based clinics.
Insured patients: Both of the bills you receive are subject to the terms, conditions and exclusions of your individual insurance policy. The bills may be subject to separate co-pays, deductibles and/or coinsurance, as defined by your insurance policy. Please contact your insurance company for detailed information regarding specific coverage.
Medicare patients: Both of the bills you receive may be subject to any applicable Medicare deductible and/or coinsurance provisions. With that in mind, you may be responsible for a portion of both bills. This amount is determined by Medicare and is based on the services provided.
