Clinical rotations

At Edward-Elmhurst Health, our goal is to foster an excellent learning experience for students. We’re proud of the outstanding patient care and compassionate nursing synonymous with Edward-Elmhurst Health. Please reach out to the Medical Staff Office for physician or PA rotations.

Edward-Elmhurst Health clinical rotations

We offer clinical rotation experiences as part of a healthcare program in coordination with select, contracted schools.

Undergraduate rotations

The following is information for undergraduate nursing, medical assistant, certified nursing assistant rotations, and additional healthcare professions.

For school coordinators

To request a clinical rotation at either Edward Hospital, Elmhurst Hospital, Linden Oaks Hospital (behavioral health) or in the Endeavor Health Medical Group and Immediate Care and walk-in clinic, school coordinators should complete and email the School Clinical Rotation Request form to the appropriate coordinator at the below email addresses:

Preferred dates for receiving clinical rotation requests are:

Fall rotation – March 1st
Spring rotation – August 1st
Summer rotation – August 1st

STEP 1

Once you are notified your rotation has been accepted, the following information will need to be submitted at least three weeks prior to the start of the rotation.  Failure to provide this documentation will cause a delay in your students being able to start their rotation.

You may request a large secure file transfer by emailing your coordinator above. 

STEP 2

To be completed by the School Coordinator:

Memorandum on school letterhead confirming that the below health requirements are file with the school for faculty and students including:

  • Current CPR provider status
  • Criminal Background check within last 90 days before rotation begins
  • Drug screen within last 90 days before rotation begins
  • HIPAA and OSHA training verification
  • N95 Mask Fit Testing – please list specific mask brand/type and date of most recent mask fit (NOTE: Mask Fit Testing is only required if students will be caring for COVID-19 positive or TB patients)
  • State of Illinois nursing licensure (graduate students only) 
  • TB test within last 12 months 

In addition to the memorandum on school letterhead the following vaccinations must be submitted on the Non-Employee Vaccine Attestation Form:

  • Measles, mumps, rubella
  • Varicella
  • T-Dap
  • Flu vaccine if clinical time is between October 1st and March 31st

For students requesting a medical and or sincerely held religious exemption, please contact the appropriate coordinator for further actions.

For faculty

To be completed on or before your first unit clinical rotation:

Faculty and students must review the Orientation Handbook (Edward Hospital) or Orientation Handbook (Elmhurst Hospital) along with the Code of Conduct bookmarked pages.  Please scan and email signed Orientation Handbook Signature Statement to the appropriate email listed above.

Post-Rotation forms

At the end of each rotation, please have the students complete and submit the Student Clinical Rotation Evaluation form and return to the appropriate email above.

If you or your students would like to recognize a staff nurse who exceeded your expectations for the clinical rotation, complete and submit the Excellence in Clinical Teaching form  and return to the appropriate email above.

Graduate rotations

To request a clinical rotation, please complete the Student Clinical Experience Request Form and email it to the site you are interested in at:

Priority is given to Edward-Elmhurst Health employees for placement.

    If you are notified of a placement, review the Student Orientation Manual for Edward Hospital or Elmhurst Hospital and Code of Conduct document.

    At least two weeks before your first clinical day, sign and return the Orientation Handbook Signature Statement – Edward and Elmhurst Hospitals to the appropriate addresses below:
